Health Hub

Health Hub is an innovative “holistic” program for connecting uninsured patients with health care and other assistance.

Typically clients who lack health insurance also have needs in other areas. Rather than subjecting them to an exhausting, duplicative and discouraging search through many agencies, Health Hub assigns clients an “advocate” to assist.

Health Hub Advocates are available to help uninsured patients find community resources to help meet healthcare needs. Advocates help clients:

  • Find a doctor/medical home
  • Access free or discounted medications
  • See a medical specialist
  • Apply for programs such as Supplemental Nutrition Assistance Program (food stamps), General Assistance, Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Medicare and Medicaid
  • Refer people to agencies designed to address basic needs.

Translation services are also available.

Health Hub is funded by grants from the Community Health Endowment (CHE) and the Catholic Health Initiative (CHI). Community partners include the Lancaster County Medical Society, People’s Health Center, the Lincoln-Lancaster County Health Department, Clinic with a Heart, People’s City Mission, ED Connection and Matt Talbot Kitchen and Outreach.
